Please fill out this form if you are eligible for the Death With Dignity Act and would like volunteer support throughout the qualifying process, need help finding participating physicians, or would like support the day of your planned death.

If you are only seeking support on the day of your planned death, please notify us 4-5 days before your planned death day, volunteers are busy with caseloads and need to have as much advance notice as possible to ensure their attendance. Even if you do not have a planned death date in mind, but know you will want volunteer support, it is best to connect with a volunteer right away.
